Job Description

Agincare are looking for a Care Coordinator to support our Live-in Care Team based within South West office near Weymouth, Dorset.

Job Role: Care Coordinator  

Hours: Permanent Contract, 40 hours per week

Salary: £22,000 - £23,000 per year, depending on experience

About the role:

The main purpose of the Care Coordinator is the responsibility of matching our carers to our clients across our South West. You will be responsible for all tasks related to helping with new client starts including matching and scheduling carers to clients.

  • You will support forward planning of scheduling and ensure it is consistently achieved in order to provide continuity of care;
  • You will need to understand the arrangements for ensuring all clients are safeguarded against the risk of abuse and respond appropriately to any allegations of abuse, or complaints regarding service delivery;
  • You will ensure that any communication with external sources such as next of kin, GP’s, nurses, physiotherapists, and any other professionals involved are prioritised if they impact on the care and wellbeing of the Client, keeping the Registered Manager updated as appropriate;
  • You will ensure that all Care Worker holidays, sickness, unplanned absences from placement, and all emergency calls are documented electronically;
  • To be part of the on-call rota system during evenings and weekends as and when required.

About you:

  • You will need to have a great level of attention to detail gained in a previous administrative or customer service orientated role;
  • Full UK driving licence required 
  • You will be spending a lot of time at your computer so you’ll need a good level of IT knowledge, in particular MS Office skills;
  • You will need to be highly self-motivated, demonstrate integrity, trustworthiness, reliability;
  • You will need to demonstrate that you have built and maintained great working relations with clients and colleagues alike;
  • Previous experience working within social care is preferable, but not essential;
  • Experience in coordinating rotas or schedules is also desirable, but not essential.
