Supporting People

Our services enable individuals to remain in their own homes, help reduce hospital admissions and contribute to a reduction in social isolation by signposting and encouraging older people to access local activities and gain very practical, targeted information, support and advice.

The service work closely with and compliments existing services for older people such as home care services and the use of alarms/telecare. This practical support provides assistance with relatively minor things such as shopping, domestic support, taking down and re-hanging curtains. However other service include a focus on crisis intervention, where there are risks of homelessness or requirements to move into residential environments where this can be prevented by accessing our support people provision.

Our key service aims and objectives.

  1. To deliver preventative outcomes that enable older people to remain in their own homes and chose where to end their lives and so avoid residential care, nursing care or hospital, where possible.
  2. To ensure service users’ are involved in all appropriate decisions which are made about their support, including intensity, frequency of visits and duration of care and support.
  3. To provide support services that enable older people to maintain their independence and achieve positive outcomes in the context of economic wellbeing, enjoyment and sense of achievement in life, physical and mental health and well-being, safety and self-determination.
  4. To monitor the well-being of service users on a regular basis and to respond effectively to any significant changes in their circumstances or well-being.
  5. To work closely with organisations in both the statutory and the voluntary sectors, to ensure service users have access to the full range of services and resources available to them.
  6. Providing service users with access to information across a range of issues, to maximise their scope to make informed decisions in respect to living independently.
