This case study details our care initiative in partnership with Worcestershire Council. By proactively securing a dedicated team of professional care workers, we provided a seamless, immediate solution for families in underserved areas. This approach ensured that individuals could return to the comfort of their own homes with 24/7 support, giving families absolute peace of mind while long-term arrangements were put in place.
The challenge
When a loved one requires significant support to live safely, families and commissioners often find themselves with limited choices in rural locations.
- The Travel Barrier: Traditional care providers often struggle to service remote areas due to the recruitment challenges and the travel times required for short daily visits, leaving a severe shortage of rural care providers.
- Discharge Delays: Without guaranteed support at home, individuals are frequently forced to stay in the hospital far longer than necessary, delaying their recovery.
- Uncertain Timelines: Families and coordinators face immense pressure to find immediate care but are often hesitant to sign restrictive, long-term contracts when they don’t yet know what their long-term needs will look like.
Our response
To solve the rural workforce shortage, from 2021 to 2026, we worked with Worcestershire Council to establish a dedicated team of experienced care workers who move into the home, eliminating travel barriers entirely.
- Rapid 7-Day Deployment: We mobilised a specialised team of 7 care workers, stepping in to deliver full-time support within just 7 days.
- Ready on Demand: Our care workers were secured and ready to be placed the exact moment each person was cleared to leave hospital.
- Total Contractual Flexibility: To ease the financial pressure on local authorities, care was delivered on a flexible, week-to-week basis.
- Targeting Underserved Areas: We focused our efforts strictly on geographical areas where traditional home care providers had no capacity.
- A Safe Bridge Home: The primary focus was keeping individuals safe, comfortable, and cared for in their own homes until a permanent, long-term care routine could be established.
Key outcomes
Over the course of a full year, this flexible model proved to be a lifeline for hundreds of individuals, providing stability and professional care when local infrastructure fell short.
- 258 Individuals Supported Safely at Home: Over a 52-week period, 258 people were successfully discharged from the hospital and welcomed back to their own homes under our care.
- The Perfect Transitional Window: People received an average of 5.7 weeks of dedicated care, providing the ideal window for recovery and future planning.
- 100% Successful Transitions: Every single individual supported was able to transition smoothly to their next step, whether moving to a permanent live-in care worker, a tailored domiciliary visiting care routine, or a residential care home.
- Zero-Risk Continuity: The flexible structure allowed families and care coordinators to maintain seamless continuity of high-quality care without any long-term contractual risks.
